Advance Care Directives
An Advance Care Directive is a document prepared in advance to set out your wishes regarding future health care. You do not need to be unwell to make one; however, it is an important document to have in place should a time arise when you are unable to make or communicate decisions about your medical treatment.
For example, if you were unconscious or otherwise incapable of giving instructions about your health care, medical practitioners would refer to your Advance Care Directive. In that document, you may have appointed a trusted person — such as a family member or close friend — to make decisions on your behalf. This ensures that decisions relating to your treatment reflect your wishes and values.
For these reasons, an Advance Care Directive is an essential part of future planning. If you require assistance in preparing your Advance Care Directive,
